29 CFR 1910.28(a)(12): An access ladder or equivalent safe access was not provided on a scaffold used by employee(s).  The employer did not furnish a ladder or equivalent safe means of access to/from the work platform(s) for employee(s) working atop a supported scaffold of tubular-welded frame construction.  Employee(s) climbed the frame leg(s), which did not meet the design uniformity and/or spacing criteria for ladder rungs.  Condition(s) were noted on or about 5/10/2014.

29 CFR 1910.28(d)(7): Guardrails not less than 2 x 4 inches or the equivalent and not less than 36 inches or more than 42 inches high, with a mid-rail, when required, of 1- x 4-inch lumber or equivalent, and toeboards, shall be installed at all open sides on all scaffolds more than 10 feet above the ground or floor.   a) Employee(s) were observed performing exterior brick cleaning at the front of a single-family house at 577 New Durham Rd in Metuchen NJ.  The crew worked from a wood plank platform on the 3rd tier of a frame scaffold, exposed to falls of between 11 and 15 ft due to lack of guardrail(s) on all exposed edges of work platforms.  Condition(s) were noted on or about 5/10/2014.
