29 CFR 1926.451(b)(3): Where guardrails were not installed along the front edge of the platform or all employees were not protected from falling with the use of personal fall arrest systems, the front edge of the scaffold platform was more than 14 inches: On or about 6/24/2014, at the above addressed jobsite, an employee was working from a two-tier scaffold removing plywood from a window whereas the front edge of the scaffold platform was more than 14 inches from the house, which there were no guardrails installed on the scaffold where the employee was working from.
