29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).Note: Specify distances as indicated in table in paragraph (f)(6) of this section. On or about July 8, 2014 employees were exposed to an electrical shock and or electrocution while erecting a metal frame scaffold with in 10 feet of an energized distribution line. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On July 8, 2014 employees walking/working from the roof of a commercial roof were not protected from a fall, exposing employees to a fall of over 25 feet. No abatement documentation or certification is required for this item.
