29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.    a.     On the north side of the site between Area 3 and Area 4 there was a hole which measured 19 inches by 19 inches which was not protected by a cover thereby exposing employees to a trip/fall hazard.    b.     On the north side of the site between Area 3 and Area 4 there was a hole which measured 50 inches by 50 inches which was not protected by a cover thereby exposing employees to a trip/fall hazard.
