29 CFR 1910.38(e):  The employer did not designate and train a sufficient number of persons to assist in the safe and orderly emergency evacuation of employees:  (a) At the newspaper operations warehouse located at 203 Kelsey Lane, Suite G, Tampa, FL 33619 employees were exposed to a fire hazard and entrapment in that employees were not trained in safe and orderly emergency evacuation.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) For employees working inside the newspaper warehouse located at 203 Kelsey Lane, Suite G, Tampa, FL 33619 as observed on or about 07-09-2014 the fire extinguisher at the Southwest corner of the warehouse was not mounted exposing the employees to a fire hazard.
