29 CFR 1926.351(b)(1): All arc welding and cutting cables were not completely insulated, flexible type, capable of handling the maximum current requirements of the work in progress, taking into account the duty cycle under which the arc welder or cutter is working. East side of the building; an employee welded an angle iron with a Hobart Champion 4500 welding machine that had a frayed lead cable with exposed copper wire, exposing employees to electrical hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. East side of the building; an employee welded an angle iron from the bucket of a Caterpillar 246C Skid Steer Loader, at an elevated height that exceeded 6 feet, exposing employees to fall hazards.
