29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks:  1) Maintenance Shop Area - Employees were using a Westward 8' drill press (model number 4TM69 and serial number W0305551) that was not equipped with a guard, exposing employees to a serious laceration hazard from rotating bit and chuck.  2) Counter Department - Employees were using a Jet drill press (model number 13-R and serial number 48744) that was not equipped with a guard, exposing employees to a serious laceration hazard from rotating bit and chuck.  3) Woodshop Area - Employees were using a Delta 16' drill press (model number 79312888152 and serial number 493167) that was not equipped with a guard, exposing employees to a serious laceration hazard from rotating bit and chuck.  4) 40H Department - Employees were using a Dayton drill press (model number 3Z917A) that was not equipped with a guard, exposing employees to a serious laceration hazard from rotating bit and chuck.  5) Pan Room - Employees were using a Wilton 6S drill press (model number 3816 and serial number 31568) that was not equipped with a guard, exposing employees to a serious laceration hazard from rotating bit and chuck.  6) Glass Shop - Employees were using a Dayton 15' drill press (model number 3Z993E and serial number 100721) that was not equipped with a guard, exposing employees to a serious laceration hazard from rotating bit and chuck.  7) Color Point - Employees were using a Orbit drill press (model number OR-1412 and serial number 6867) that was not equipped with a guard, exposing employees to a serious laceration hazard from rotating bit and chuck.

29 CFR 1910.134(c)(2)(i): The employer did not provide respirator users with information contained in Appendix D to this section("Information for employees using respirators when not required under this standard") when voluntary respirator use was permissible:  1) Production Department - An employee voluntarily using a 3M N95 filtering facepiece (model  # 8210 and serial # 440060) two strap dust masks were not provided with the information contained in Appendix D of the respirator standard.  2) Shipping and Receiving Department - An employee voluntarily using a 3M N95 filtering facepiece ( model # 8210 and serial # 440060) two strap dust masks were not provided with the information contained in Appendix D of the respirator standard.
