29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:  a- 1405 Barnegat Lane, Mantoloking NJ: Company employees were exposed to fall hazards of approximately 21 feet 5 inches to the ground surface below when accessing and egressing the roof of a residential structure using a fiberglass extension ladder whose feet were sinking into the sand below and the ladder was not secured, at any other point, against accidental displacement.  Condition observed on or about 07/10/14.

29 CFR 1926.1053(b)(15): Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use:  a- 1405 Barnegat Lane, Mantoloking NJ: Company employees were exposed to fall hazards of approximately 21 feet 5 inches to the ground surface below when accessing and egressing the roof of a residential structure using a fiberglass extension ladder that contained visibly damaged components and whose feet were sinking into the ground where it was set. Condition observed on or about 07/10/14.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a- 1405 Barnegat Lane, Mantoloking NJ: Company employees were exposed to fall hazards of approximately 21 feet 5 inches to the ground surface below when accessing and egressing the roof of a residential structure using a fiberglass extension ladder that contained visibly damaged components.  Condition observed on or about 07/10/14.
