29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level: a- 1405 Barnegat Lane, Mantoloking NJ: Company employees were exposed to fall hazards of approximately 21 feet 5 inches to the ground surface below when accessing and egressing the roof of a residential structure using a fiberglass extension ladder whose feet were sinking into the sand below and the ladder was not secured, at any other point, against accidental displacement. Condition observed on or about 07/10/14.

29 CFR 1926.1053(b)(15): Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use: a- 1405 Barnegat Lane, Mantoloking NJ: Company employees were exposed to fall hazards of approximately 21 feet 5 inches to the ground surface below when accessing and egressing the roof of a residential structure using a fiberglass extension ladder that contained visibly damaged components and whose feet were sinking into the ground where it was set. Condition observed on or about 07/10/14. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 896053, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON 06/12/13, WITH RESPECT TO A WORKPLACE LOCATED AT 1402 WASHINGTON AVENUE, SPRING LAKE NJ.
