29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:    Worksite: When using a generator to operate pneumatic nail guns, the employer did not instruct and train employees in the hazards of anoxic conditions in enclosed areas.  The employer set up and operated a gasoline-powered generator in the below-ground basement of the house under construction.  This allows the accumulation of Carbon Monoxide and other combustion by-products in the basement.  If left uncorrected this could cause asphyxiation and serious injury to any worker entering this area.
