OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards when walking and stepping on roller conveyors:     a.  On or about June 24, 2014, in the finished product area of MEP#2, employees were not protected from slip and fall hazards or being caught-between when stepping and walking over free standing roller conveyor to tie off and strap bundles of standard wire mesh.    b.  On or about June 24, 2014, in the finished product area of QC-7, employees were not protected from slip and fall hazards or being caught-between when stepping and walking over motorized roller conveyor to tie off and strap bundles of standard wire mesh.      c.  On or about July 16, 2014, in the feed table area of QC-7, employees were not protected from fall hazards or caught-by when stepping and walking over motorized roller conveyors to spread wire rods to feed the machine.

On or about June 24, 2014, at the north bay of the manufacturing plant, the plug-in cord of the main monitor panel of MEP#1 run through a metal floor cover out to wall outlet, exposing employees to electrical hazards.

29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident:  a.  On or about June 24, 2014, at the manufacturing plant, employees involved in near miss incidents with powered industrial trucks were not provided with refresher training to prevent a struck-by hazard.
