29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:   a) 56 Westfield Avenue, Clark, NJ 07066. The employer allowed his employee to work on and from tubular welded scaffold when the walking/working platforms were not fully planked exposing the employee to fall hazards.  Condition occurred on or about 07/12/14.

29 CFR 1926.451(e)(1): Scaffold platform more than 2 feet above or below point of access, was not provided with a ladder or direct access from another structure or other equivalent safe access during use:    a) 56 Westfield Avenue, Clark, NJ 07066. The employer allowed his employee to work on and from tubular welded scaffold when there was no ladder provided or direct access from another structure or other equivalent safe access during use. Employee was observed climbing scaffold components.     Condition occurred on or about 07/12/14.

29 CFR 1926.451(g)(1): Each employee on scaffolds more than 10 feet high were not protected from falling to the lower level:     a) 56 Westfield Avenue, Clark, NJ 07066. The employer allowed his employees to work on and from tubular welded scaffold with no fall protection systems in place, i.e. guardrails installed along all open sides and ends of walking/working platforms.     Condition occurred on or about 07/12/14.         Guaman Stone Stucco was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 316180728, citation number 1, item number 4 and was affirmed as a final order on 02/21/2012, with respect to a workplace located at 56 Broadway, Denville, NJ 07834.
