29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. On or about July 10, 2014 and times prior thereto, employees of Westridge Mills, Inc. performed maintenance and repair of sheller and rice huller equipment in the mill located at or near 289 Sifford Avenue in Cherry Valley, Arkansas. The employer had removed guardrails on the catwalks located greater than 4 feet from the ground level below, exposing employees to fall hazards.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about July 10, 2014 and times prior thereto, employees of Westridge Mill, Inc. performed maintenance and repair on sheller and rice huller equipment at the mill located at or near 289 Sifford Avenue in Cherry Valley, Arkansas. The employer had not established a written lock out/tag out program, exposing employees to the hazard of being caught in machinery.
