29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  1652 EAST THISTLE DRIVE, WYOMISSING PA, ROOFTOP - Employees performed roofing work, and the employees were not protected by any type of fall protection, thereby exposing the employees to fall hazards that ranged from approximately eleven (11) to thirty (30) feet to the ground below, on or about June 20, 2014.    No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  1652 EAST THISTLE DRIVE, WYOMISSING PA, ROOFTOP - Employees performed roofing work at heights ranging from approximately eleven (11) to thirty (30) feet where the employees were not protected by any type of fall protection, and the employer did not provide fall protection training to the employees, thereby exposing employees to serious injury, on or about June 20, 2014.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) 1652 EAST THISTLE DRIVE, WYOMISSING PA, SIDE OF HOUSE - Employees used a thirty-two foot extension ladder to gain access to the upper roof, and the ladder did not extend at least three (3) feet past the upper landing surface, thereby exposing employees to fall hazards of at least twenty-four feet to the ground below, on or about June 20, 2014.    No abatement certification or documentation required.

29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder were not placed with the two rails supported equally:  (a) 1652 EAST THISTLE DRIVE, WYOMISSING PA, SIDE OF HOUSE - Employees used a thirty-two foot extension ladder to gain access to the upper roof, and the rungs of the ladder were rested against the angled side of the roof so that the rails were not supported equally, thereby exposing employees to fall hazards of at least twenty-four feet to the ground below, on or about June 20, 2014.  No abatement certification or documentation required.
