OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  Drilling rig floor:  On or about July 10, 2014, and times prior to, casing crew employees working with a drilling rig crew were inserting the production casing pipe into the well.  One of the casing crew working from the stabber position on the derrick was not provided with a secondary means of emergency egress.  This exposed the employee to the hazards of explosion and/or fire, and/or being struck by pipe during a well blow out.
