29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)       First Floor - On or about 07/14/2014, workers renovating a row home were exposed to a 9 foot fall from a 5 foot by 7 foot wall opening which had not been provided with fall protection.       b)      Third Floor - On or about 07/14/2014, workers renovating a row home were exposed to a 30 foot fall from a 5 foot by 7 foot wall opening which had not been provided with fall protection.       c)       Second Floor  On or about 07/14/2014, workers renovating a row home were exposed to a 10 foot fall through a 3.5 foot wide by 20 foot long unguarded floor hole.       d)       Third Floor  On or about 07/14/2014, workers renovating a row home were exposed to a 20 foot fall through a 3.5 wide foot by 20 foot  long unguarded floor hole.       e)      Front Porch  On or about 07/14/2014, workers renovating a row home were exposed to a 8 foot fall from the open side of a concrete porch.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a)  Porch - On or about 07/14/2014, workers were exposed to a 4 foot fall hazard from a job-made ladder that projected only 18 in. above the ladder landing.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:   a)  Job Site - On or about 07/14/2014, workers using portable electric power tools, including, but not limited to, circular and reciprocating saws, were not provided with either ground fault circuit interrupters or an assured equipment grounding conductor program.
