29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   On July 15, 2014 employees were not protected from a fall while working from a ladder jack scaffold, exposing employees to a fall of approximately 18 feet above grade.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On July 15, 2014 employees were not protected from a fall while roofing a residential property, exposing employees to a fall of 19'8" above grade.   No abatement documentation is required for this item.
