29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years.    On or about 15 July 2014, and at times prior thereto, three employees who utilize the forklift had not been retrained within the last three years to determine their operating skills and validate their being able to operate the forklift, exposing them and those employees working around them to the hazard of being struck by the forklift.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 pounds per square inch (psi).    On or about 15 July 2014, and at times prior thereto, five maintenance workers were allowed to use compressed air to clean off their workstations on a daily basis that was at approximately 78 psig, exposing the workers to the hazard of being struck by flying objects or debris.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy.    On or about 15 July 2014, and at times prior thereto, the employer allowed the use of an electrical outlet that had a damaged faceplate with exposed energized parts, exposing the employees using the outlet to an electrical hazard of approximately 120 volts.
