29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: One employee was exposed to electrical shock and arc flash hazards while unearthing three wires for a 12.5 kV three phase conductors, and the employer did not provide protective clothing such as rubber insulated gloves, arc-flash clothing, face-shield, and balaclava while conducting an inspection of the three phase wire during the removal of it in the excavation.

29 CFR 1926.416(a)(2): In work areas where the exact location of underground electric power lines was unknown, employees using jackhammers, bars, or other hand tools which may contact a line were not provided with insulated protective gloves: Employees were exposed to possible electrical shock while unearthing a three phase 12.5 kV electrical line with hand shovels, and the employees were not provided with or wore insulated protective gloves.
