29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  Alumni Roofing Co., on or about July 15, 2014, at the address of 715 Richland Mall, Mansfield, OH: The employer failed to ensure employees tasked with the installation guardrail systems around the commercial structures tower were protected from falls during construction. Employees tasked with erecting the protective systems were not attached to fall protection ropes attached to the tower, exposing the employees to a fall of approximately 85 ft. to the ground below.

The employer failed to evaluate the adequacy and certify fall protection training that employees had participated through a prior employer.
