29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   Worksite located at I-40 bridge and Garth Brooks Blvd. Yukon, OK 73099. On or about July 15, 2014 employees were observed working from an aerial lift without the use of a fall protection harness and lanyard. The employees were exposed to a fall hazard of approximately 10 feet.
