29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:      The employer does not ensure that a body belt is worn and a lanyard attached to the boom or basket when working from an aerial lift.  This violation was observed on or about June 15, 2014, on the southwest side of the building where employees were exposed to a fall hazard when installing foam trim from a Genie S-40 (SN: S4006-11381) aerial lift without a fall protection system attached to the boom or basket.        The Theut Glass Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 314167875, citation number 1, item number 1, and was affirmed as a final order on March 16, 2010 with respect to a workplace located at 19636 Plantation Myrtles Drive, Katy, Texas 77449.        Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a body belt is worn and a lanyard attached to the boom or basket when working from an aerial lift.
