29 CFR 1910.305(g)(1)(iv)(A):  Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   a) Receiving room, 2297 Curryville Road, Martinsburg, PA 16662:  Observed on or about July 09, 2014, one green and one yellow flexible cords (one green, one yellow) were being used as a substitute for the fixed wiring of the building.  The green cord was draped over an alarm bell above the door leading from the receiving room to the store.  The yellow cords was used to supply power to various appliances including a fan, microwave, and refrigerator.  The green cord was plugged into a wall outlet (110 volt) to the right of the door.  Employees working in the receiving room were exposed to the hazard of electric shock.

Employees were exposed to burn and smoke inhalation hazards.  b) Receiving room, left of emergency exit door, 2297 Curryville Road, Martinsburg, PA 16662:  Observed on or about July 09, 2014, a Badger brand portable fire extinguisher was not accessible and the location of the portable fire extinguisher was not identified.  Access to the portable fire extinguisher was restricted by boxes, a broom, and metal shelving stored in front of the fire extinguisher.  Employees were exposed to the burn and smoke inhalation hazards.    c) Store area, left of door leading into receiving room, 2297 Curryville Road, Martinsburg, PA 16662:  Observed on or about July 09, 2014, a Badger brand portable fire extinguisher was not accessible and the location of the portable fire extinguisher was not identified.  Access to the fire extinguisher was restricted by materials stored in front of the fire extinguisher.  Employees were exposed to the burn and smoke inhalation hazards.
