29 CFR 1926.451(b)(3): Where guardrails were not installed along the front edge of the platform or all employees were not protected from falling with the use of personal fall arrest systems, the front edge of the scaffold platform was more than 14 inches:    a) Exterior left side wall: Employees without any means of fall protection in place were engaged in stucco activities while working from a pump jack scaffold platform that was approximately 24 inches from the working surface being exposed to a fall of approximately 15 feet, on or about 5/19/14.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   a) Right side wall: Employee engaged in stucco activities was accessing the pump jack scaffold platform by climbing a 2 frame high welded tubular scaffold without the use of a ladder, being exposed to a fall hazard, on or about 5/31/2014.
