29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:   Storage area:  Employees were exposed to falls of approximately 8-feet, 3-inches, while loading and unloading materials between the storage mezzanine and a forklift with the guardrail removed.  Additionally, the guardrails in the loading areas were not effective because they were not equipped with a mid-rail.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Turn-Pro 7-inch metal cutting band saw:  Employees were exposed to amputation and laceration hazards while operating the band saw where the unused portion of the blade below the table was not guarded.  b)  Wilmarth & Morman Co. belt grinding machine:  Employees were exposed to amputation and laceration hazards while operating the belt grinding machine where the spindle ends and nuts on the pulleys for both the left and right belts were not guarded and the in-going nip points between the grinding belts and the pulley were not guarded.
