The employer's written Bloodborne Pathogen Exposure Control Plan was not site specific in that, the written plan did not identify the following:     1.) Specific information regarding the healthcare professional that will conduct the confidential post exposure evaluation and follow up in the event of an exposure incident;      2) Procedures for the annual evaluation of safety engineered devices.  The method of implementation was not described, nor was a start date identified;        b) South Central Alpha Housing, Inc. dba Avalon Nursing Center, 3410 W Pittsburg Rd, New Castle, Pa, On or about July 17, 2014, and times prior thereto, The section and associated forms of the facility's Exposure Control Plan titled "Bloodborne Pathogens Post Exposure Evaluation & Followup"  and the related consent form contained information that would not protect the confidential medical information of the employees receiving the Post Exposure Evaluation and Follow-up.
