29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was each employee provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):      (a) On or about July 15, 2014, and at times prior, for the employees sheeting a 4X12 pitch roof and exposed to falls of approximately 10 feet, 7.5 inches, off Highway 240 south of Wall, South Dakota.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:      (a)  On or about July 15, 2014, and at times prior, for the employees engaged in residential framing activities and exposed to electrical hazards while using an extension cord with outer cover burned through the insulation off Highway 240 south of Wall, South Dakota.
