Scaffold legs rested on concrete debris and lumber.

29 CFR 1926.452(a)(2): Crossbracing shall be installed between the inner and outer sets of poles on double pole scaffolds:    On or about July 16, 2014, at a residential structure located at 2027 Cupressus Court, Allen, Texas, 75013; an employee working from a scaffold when doing masonry work was exposed to fall hazards. Employee was not protected by the use of fall protection systems. Crossbars and crossbracing were not installed on both sides of scaffold.
