On or about 07/17/2014, at the job site - the use of personal fall protection was not enforced while operating an aerial lift to install PVC siding, exposing the worker to a 28 feet fall hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a. On or about 07/17/2014, at the job site - employees were exposed to a 20 ft fall hazard, in that, fall protection was not used while installing PVC siding on a 7:12 sloped roof.
