29 CFR 1926.501(b)(11): Steep roofs. Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:       The employer did not ensure appropriate fall protection was implemented for seven (7) employees performing commercial roofing construction to replace the roof on an assisted living facility.  Employees were exposed to fall hazards associated with an approximately thirty (30) foot fall while working on a 6/12 pitch roof.       The employer (Shrum, Inc) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 3133018939, citation number 01, item number 3a and was affirmed as a final order on 05/25/2011, with respect to a workplace located at Unit G East Meadow Ridge, Columbia, IL  62236.
