29 CFR 1910.36(d)(1): Employees were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors:    a)  On or about July 15, 2014, the exit route door that separates the retail space from the storage room was  latched and padlocked closed while employees were in the store.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a)  On or about July 15, 2014, in the dressing room lobby, a portable fire extinguisher mounted on the wall did not have signage indicating its location.
