Location: Production Floor  Employees were exposed to laceration hazards while operating the Baldor Grinder Buffer where the adjustable tongue guards for the right and left abrasive wheels were not adjusted to within one-quarter inch of the abrasive wheel.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval.  Location: Production Floor   No written approval from the forklift manufacture was obtained by the employer to use the Caldwell PB-60 adjustable boom attachment with the Hyster forklift.  The addition of the boom attachment decreases the capacity of the forklift.
