On or about 22 July 2014, and at times prior thereto, three fire extinguishers were not clearly marked or identified so that they could be readily accessible to employees, exposing the employees in the area to a fire hazard. 1) The extinguisher located in the Bulk Feed Room was not identified with a sign. 2) The extinguisher located at the Contractor Entrance was not identified with a sign. 3) The extinguisher located inside the Breaker Room was not identified with a sign.

29 CFR 1910.303(g)(1)(i)(A): The depth of the working space in the direction of access to live parts was less than indicated in Table S-1, or 36 inches for 600 volts, nominal, or less. Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed. On or about 22 July 2014, and at times prior thereto, electrical panels were blocked by various items in front, exposing employees to an electrical shock hazard. 1) Two electrical breaker panels, a knife switch, and a transformer located in the southwest corner of the warehouse were blocked for access by various items. 2) Two electrical panels located near the lumber yard office were blocked for access by various items.
