29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:      a)  Worksite, 2995 Middle Country Rd., Lake Grove NY - Employee was installing plywood on the roof of a residential complex.  The employee was approximately 30ft above the ground and was not tied off to the basket; on or about 7/23/14.       Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
