29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators.                                                                     Worksite: The ladder type material hoist that was used by employees to gain access to the roof at a height of approximately 16 feet above the ground was not designed to be used in such a manner.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).    Worksite: The employee who was working on 6 to 8 pitch roofs with eve heights up to 28.5 feet above the ground, was not protected from fall hazards by the use guardrails, safety nets or a personal fall arrest system.      Isas Construction, LLC (now Four Elements Restoration, LLC) was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 941508, citation number 1, item number 2a, issued on 9/30/2013 with respect to a workplace located at 19-27 Commodore Commons Court, Derby, CT and became a final order on 11/19/2013.
