Location rear of the project at 338 Humbolt Street, Brooklyn, NY   On or about 7/24/2014  a). Employees were utilizing a Dewalt brand portable 10 inch table saw at the rear of the project to rip materials as needed to erect a wood framed deck. Employees were not protected from amputation hazards because the manufacturer supplied self adjusting blade guard had been removed from the saw.

Location: 338 Humbolt Street, Brooklyn, NY front of project.  On or about 7/24/2014  a). Employees were removing siding form the front of the structure working from an elevated pump jack scaffold system about 14 feet above the ground and were not protected from fall hazards because the required bottom supports were not installed as per manufacturer specifications.
