29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  Worksite located at 1765 Shedeck Parkway, Yukon, OK 73099. On or about July 23, 2014, employees were observed working from scaffolding without the use of fall protection.  The employees were exposed to a fall hazard of approximately 14 feet.

On or about July 23, 2014, employees were observed working in area where there was a danger of injury from falling bricks or tools and were not in use of protective helmets.  The employees were exposed to struck-by hazards.
