29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.   Workers were engaged in residential construction activities re-roofing an existing single-family house. Workers were exposed to potential fall hazards from a height of 9.0 feet and a inspection of the jobsite by a competent person to ensure that the jobsite, materials, and equipment such as fall protection systems were maintained and utilized in a safe condition was not conducted.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   Workers were engaged in residential construction activities re-roofing an existing single-family house. Workers were exposed to potential fall hazards from a height of 9.0 feet in that a proper fall protection system was not utilized.
