29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Jobsite: On or about July 24, 2014, the ground man was not wearing a hard hat while he was working below employees who were working on the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jobsite: On or about July 24, 2014, two employees were working from a roof on a residential home and they were not protected from falling to the ground. The fall hazard to the two employees was about 21 feet. The pitch of the roof was 10 in 12.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: Note: Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used render previous training obsolete; or (3) Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill: (a) Jobsite: On or about July 24, 2014, two employees were working on a roof and they were not protected from falling about 21 feet to either the ground or to pavement. The two employees need to be retrained to be able to recognize fall hazards and the methods to be used to minimize those hazards. The pitch on the roof that the two employees were working on was 10 in 12.
