29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about July 21, 2014, the employer failed to ensure employees using a Bil-Jax Aerial Lift, Model 5533A to perform tree trimming operations were protected against a fall of approximately 25 feet by requiring the use of a personal fall arrest system.
