29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified: (a) On or about July 28, 2014, and at times prior, employees were exposed to skin irritation when lacquer thinner containing toluene, xylene, methyl isobutyl ketone and methanol when five millimeter Ansell TNTnitrile gloves, Radnor nitrile examination gloves and Diamond Grip Powder Free Latex gloves broke through.

29 CFR 1910.134(h)(3)(ii)(A): Respirator inspections did not include a check of respirator function, tightness of connections, and the condition of the various parts including, but not limited to, the facepiece, head straps, valves, connecting tube, and cartridges, canisters or filters: (a) On or about August 14, 2014 and at times prior, Sata Vision 2000 supplied air hood inlays were not maintained per the manufacturer's design which would have been discovered during an inspection.
