29 CFR 1910.269(a)(3): The employer did not determine existing conditions related to the safety of the work to be performed before work on or near electric lines or equipment was started: On or about 13 June 2014, Naval Base Point Mugu's Department of Public Works failed to confirm that the A3-T211 is a 150KVA 4160-120/208V 3 Phase delta/wye dry pack transformer was free of corrosive residue. Corrosive foreign material residue was present where internal fuses make contact on the lower bus, exposing employees to flash arc, electrical shock and electrocution hazards.

29 CFR 1910.269(d)(6)(iv): Lockout or tag out devices were not affixed to each energy isolating device by authorized employees: On or about 13 June 2014 the Employer failed to ensure that lock out or tag out devices were used in order to secure the primary side of A3-T211 transformer with either a 5 point head screw needing a specialized tool, or a tag to ensure high side is not accessible by unauthorized personnel in which an employee made contact with a closed circuit.
