29 CFR 1910.178(q)(6): Industrial trucks shall not be altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, nor shall they be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section. Additional counterweighting of fork trucks shall not be done unless approved by the truck manufacturer.  a. At the site, the fork tine on the Komatsu forklift was altered without prior consent from the manufacturer. The tine had a hole drilled through it and a ball hitch was attached by a nut and bolt through the tine.

When an injury or illness involves restricted work or job transfer but does not involve death or days away from work, you must record the injury or illness on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and an entry of the number of restricted or transferred days in the restricted workdays column.  Located at ECM Energy Services in Cambridge, OH:  a. On the 2014 OSHA 300 log, the employer did not ensure the number of days for restricted or transferred was entered in column I for case numbers 2, 3, and 7.

You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at ECM energy Services, Inc. in Cambridge OH:  a. On the 2014 OSHA 300 log, the employer did not ensure all the required information was included in column F. The entries in column F did not include the actual injury and the object/substance that caused the injury.
