29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: Location: 3424 Lasalle Street, St. Louis, MO 63104 At the job site, the employer failed to ensure the all electrical power equipment (power tools & flexible cords sets) were visually inspected for external defects prior to use. Visual inspection of the flexible cords revealed insulation damage and a missing ground pin. The workers were exposed to an electrical hazard.
