29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about July 29, 2014; at the center of the jobsite, the employer allowed employees to utilize a floor fan with a plug that was missing a ground pin. The floor fan was used to cool the work area. The missing ground pin prevented the floor fan from having a continuous path to ground.
