29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were the employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a)Guerrero, Criso at 17208 E 109th Way, Commerce City, CO 80022: On and before July 19th, 2014, the employer did not ensure that employees were using fall protection when shingling a two-story residential roof with a 6 on 12 pitch. The condition exposed employees to fall hazards of up to approximately 22 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surfaces being accessed: (a)Guerrero, Criso at 17208 E 109th Way, Commerce City, CO 80022: On and before July 19th, 2014, the employer did not ensure that the ladders used to gain access to the lower and upper roofs were extended up at least 36 inches. The condition exposed employees to fall hazards of up to 12 feet 10 inches.
