29 CFR 1910.67(c)(2)(v): A body belt/harness was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  a) On or about 07/29/14, on jobsite at 191 Delaware Ave. & W. Huron St, Buffalo, NY: Two employees were exposed to potential fall of approx. 16 ft. from the elevated "basket" of a "JLG  Model 40 HA aerial lift, while removing a speaker from a lamp post. Neither employee was wearing a body belt/body harness with a lanyard, and neither employee was tethered to the basket/boom in any other way.    NO ABATEMENT CERTIFICATION REQUIRED
