B)     Swartz Manufacturing, Inc., located at 820 Walnut Street, Elyria, OH 44035: On or about May 29, 2014, the employer failed to develop, document, and utilize machine-specific energy control procedures to control hazardous energy of equipment such as, but not limited to, Brother TC-321 CNC Drilling and Tapping Center (located in the front). Employees did not control all hazardous energy sources such as, but not limited to, electrical and pneumatic, while performing activities such as, but not limited to, changing tooling. Employees were exposed to amputation hazards.
