29 CFR 1926.502(d)(15)(i): The Employer did not maintain a safety factor of at least two as part of a complete fall arrest system:  Location: Jobsite  Employees were exposed to fall hazards while working at a height greater than 6 feet, where the Employer did not maintain a safety factor of at least two as part of a complete fall arrest system.  Instance 1) Only two anchors were available for use by 4 employees. One employee connected his VLL to the same anchor another Employee was connected to.  Instance 2) One anchor did not have the required number of screws or nails to properly attach it to the roof. This same anchor was attached through several layers of asphalt shingles.
