Throughout Facility- On or about July 30, 2014, employees were required to use ladders that were not in serviceable condition. These ladders were neither tagged or taken out-of-service. Employees were thereby exposed to fall hazards.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    a. Throughout Facility- On or about July 30, 2014, employer did not develop an Personal Protective Equipment (PPE) assessment designed to protect employees from hazards associated with tasks they were required to perform inside trailer yard. Thereby exposing employees to head, face, and struck-by hazards.
