29 CFR 1926.1404(a)(1): Assembly/disassembly must be directed by a person who meets the criteria for both a competent person and a qualified person, or by a competent person who is assisted by one or more qualified persons ("A/D director"):     (a) Jobsite, The Village at Camp Trees, Lot 111, Mars, PA:  On or about 07/29/14 the assembly of the jib on the National Crane Corporation mobile crane was not directed by a person who met the criteria as a competent and qualified person.  The crane operator had not received training or instruction on the procedures for the installation of the jib in accordance with the manufacturer's recommendations nor did the operator have previous work experience in this type of crane assembly thereby not meeting the criteria as a qualified and competent person.  The operator suffered head injuries when he was knocked off the crane while attempting to install the jib.  The operator did not have the proper pins available to adequately secure the jib to the boom and while attempting to move it the jib shifted and knocked the employee off the crane to the ground.

The A/D director must review the applicable assembly/disassembly procedures immediately prior to the commencement of assembly/disassembly unless the A/D director understands the procedures and has applied them to the same type and configuration of equipment (including accessories, if any):  (a) Jobsite, The Village at Camp Trees, Lot 111, Mars, PA:  On or about 07/29/14 the operator, who was in charge of the assembly of the jib on the truck mounted hydraulic telescoping crane, had not reviewed the applicable procedures prior to beginning the assembly nor was the operator familiar with this type of equipment.
